To submit your vaccine for verification, please follow the steps outlined below:
1. Log in to your ThrivePass account and select "COVID-19 Verification Center"
2. Select "Partially Vaccinated"
3. Select the COVID-19 Vaccine you received
4. Enter the dates you received your first dose, and if you are planning on receiving your final dose
If you are not planning on getting your final shot, you may be prompted to fill out an Exemption Form and upload it.
If you are planning on getting your final shot, please continue with the following instructions.
5. Enter your final shot information, which includes:
- If you plan on getting your final shot.
- If you have your final shot scheduled.
6. Enter your contact information
7. Review the "Next Steps" page
8. Review your entered information and ensure it is correct, then agree to the confirmation and select "Submit"
